HLTHPS006 (assist)
HLTHPS006 is assistance — helping a person take their own medication as set out in care plans and a health professional’s instructions. You check, prepare, prompt and record while working within your own ability and documented procedures.
Quick rule: If it sits outside the plan or your training, don’t proceed. Stop, document, report and ask.
The 7 Rights: your core safety check
Before, during and after you assist clients with medication, confirm:
Right client — check ID against the care plan and MAR
Right medication — correct name and strength; beware look-alike names
Right dose — follow the dosage instructions
Right route — oral, topical, inhaled, or as listed in the plan
Right time — correct schedule; date medication when required
Right documentation — record straight away and note outcomes
Right reason/response — aligns with directions; note the client’s response
Follow infection control guidelines: hand hygiene, clean equipment and PPE as needed.
Contingencies you must handle
Be ready for common medication contingencies:
Refusal: Respect choice. Do not force or disguise medicine. Record, notify and follow the plan.
Errors or suspected errors: Stop. Seek advice. Follow incident steps and complete reports.
Unclear labels or missing scripts: Do not proceed. Report and request clarification.
Expired or discarded medications: remove from use and follow safe and appropriate disposal rules.
Adverse reactions: Support the person, follow emergency steps, escalate and document.
PRN liquid paracetamol scenario: Check the PRN order, last dose time, 24-hour max, dosage instructions, and symptoms that allow use. If anything is unclear, don’t give it. Escalate.
