Quick rule: If it’s outside the plan or your training, don’t proceed. Stop, document, report, and ask.

The 7 Rights: your core safety check

Before, during and after you assist clients with medication, confirm:

  1. Right client â€” check ID against the care plan and MAR

  2. Right medication â€” correct name and strength; watch for look-alike names

  3. Right dose â€” follow the dosage instructions

  4. Right route â€” oral, topical, inhaled, or as listed in the plan
